This form is intended to provide feedback on a member or a member company as part of the member’s renewal process but may be used at any time to address conflicts or any potential issues that have been identified with regards to membership.

If you are trying to provide feedback on an applicant please use the applicant feedback form.

By default, this form will be submitted anonymously. If you wish to indicate your name, you may do so at the bottom of the form. Keep in mind that the membership committee cannot reach out to discuss your concerns if the form is submitted anonymously.
